  • Purchase and delivery information, such as the product purchased, payment status, transaction date, and delivery status. Payment-card details are processed by our payment provider and are not stored by Clarity Path Partners.

Please do not submit medical records, detailed health information, Social Security numbers, or other information that is not needed to request or receive the workbook.

  • Deliver the Preview Edition or a purchased digital workbook.

Submitting the Preview form authorizes the transactional email needed to deliver the requested Preview Edition. It does not by itself enroll you in SMS or ongoing marketing messages.
